Title: When conservative periodontal therapy is enough: Complete resolution of pregnancy associated gingival enlargement

Abstract:

Background: Pregnancy is associated with significant hormonal changes that can modify the gingival response to local irritants. Pregnancy-associated gingivitis is common and may occasionally present as localized gingival enlargement, commonly referred to as a pregnancy tumor or pyogenic granuloma. Although plaque and calculus are the primary local contributing factors, hormonal alterations may exacerbate the inflammatory response. Management ranges from nonsurgical periodontal therapy to surgical intervention, depending on the severity and persistence of the lesion.
Case Presentation: A 32-year-old woman presented with localized gingival enlargement that had developed during pregnancy and persisted after delivery. Clinical examination revealed gingival inflammation associated with localized gingival overgrowth. Given the clinical presentation, a conservative nonsurgical approach was initially selected.
Treatment and Outcome: The patient received comprehensive nonsurgical periodontal treatment, including professional plaque and calculus removal, oral hygiene instruction, and reinforcement of effective plaque control. No surgical intervention was performed. Progressive reduction of the gingival enlargement was observed during follow-up, ultimately resulting in complete regression of the lesion. The patient was subsequently enrolled in a supportive periodontal therapy program.
Conclusion: This case highlights that pregnancy-associated gingival enlargement may resolve completely following adequate nonsurgical periodontal management, even when the lesion persists postpartum. Effective plaque control combined with regular supportive periodontal therapy can play a crucial role in achieving and maintaining long-term periodontal stability and may help avoid unnecessary surgical intervention.
