Title: Orthodontic surgical traction of impacted teeth: From 3D diagnosis to biomechanical management
Abstract:
Background: Tooth impaction is a very common clinical case in daily orthodontic practice. Most frequently affects maxillary canines and central incisors. Tooth impaction has detrimental consequences, such as root resorption of the adjacent teeth, dental midline deviation, anterior crowding, arch perimeter shortening infection, and pain. Orthodontic placement of these teeth is highly recommended in adult patients. Therefore, successful management requires close collaboration between the orthodontist and the oral surgeon, as well as careful diagnosis and planning.
Objective: This presentation aims to outline a structured approach to the combined orthodontic and surgical traction of impacted incisors and canines.
Content: A review of the orthodontic literature was conducted focusing on clinical examination, the determining role of cone-beam computed tomography (CBCT) in accurately locating the impacted tooth. Attention was given to the biomechanical traction force, direction and point of application. Anchorage management will be examined, from conventional dental anchorage to skeletal anchorage using miniscrews.
Conclusion: A rigorous diagnosis supported by 3D imaging, sound biomechanical planning and appropriate anchorage are essential for the predictable and safe traction of impacted teeth.
